Seeds Care provide outreach services to support individuals aged 16+ in their home or in the community. We provide bespoke person-centered care packages which support individuals with anything from personal care to help with finances, to days out; helping build independence and confidence.
The level of support we offer depends on each individual and we work hard to create bespoke care packages that are tailored specifically to the individual’s needs. Outreach services can act as a stepping stone to independence and can be instrumental in effective transitions to supported living placements in the future.
Outreach services are ideal for individuals who require the familiarity and consistency of their home environment and surroundings but need some support with day-to-day activities and to help build independence.
Outreach differs from residential or supported living care as the individual still lives at home (not in full time care) and only receive support for things they personally need.
